For regular appointments, please give 24 hour notice of cancellations. Appointments scheduled to last more than 30 minutes require a 3 day prior notification of cancellations to prevent a $100 cancellation fee.

Some insurance plans require a referral from your primary care physician in order to cover a visit to a specialist, so please check with your insurance provider before your appointment.
